Nurses are subjected to compassion fatigue, burnout, high stress, and moral distress. Many nurses work 12-hour shifts within a facility with staffing shortages and are determined to balance work with multiple aspects of life outside of work. Under stress, over worked, and minimally assisted, nurses are vulnerable to making mistakes that could lead to death. â€Å"The Institute of Medicine reported that communication failures account for most adverse outcomes in hospitals† (Margo, 2013). Cleopatra Assignment What does it tell us about Plutarch’s view of the relationship between Antony and Cleopatra? Plutarch views the relationship between Antony and Cleopatra as one where Cleopatra uses manipulation, temptation and obsession to control Antony. The Romans did not value romantic love and this infatuation was another indication to Plutarch of Antony’s self indulgence and complete lack of self-control. Plutarch then adds she ‘kept him in constant tutelage and released him neither night or day’ playing dice, drinking and hunting with him showing an obsessiveness and a leading role in this relationship and never letting Antony out of her sight. His infatuation for Cleopatra was seen as a failing and Cleopatra treated Antony with disdain and mockery, all was on her terms and Antony seemed powerless to resist her charms. Antony goes to great lengths to impress Cleopatra, for example he cheats when fishing by ordering â€Å"his fishermen to dive down and secretly fasten to his hook some fish and Cleopatra goes along with this and humours him by pretending to â€Å"admire her lover’s skill†. Cleopatra then confides in her friends about Antony’s ‘skill’ and invites them to be spectator’s the following day. This attempt at impressing Cleopatra in his fishing ability backfires, as she is one step ahead. As soon as Antony has thrown in his line â€Å"she ordered one of her own attendants to get the start of him by swimming onto his hook and fastening on it a salted Pontic Herring†. When Antony pulled in his catch revealing the kipper, which is not an impressive catch, this was met with great laughter by Cleopatra’s spectators and made Antony to look like a fool. It must be noted that this is a viewpoint from Plutarch from a series of character studies and not designed as a piece of conventional narrative history. Here are several sentences that benefit from rearrangement. 1. â€Å"Complex adaptive systems can respond more quickly the more complex they are.† This sentence is clear and straightforward, but it would be stronger with a shift in emphasis. What’s the key point? Speed increases as complex adaptive systems become more complex. This revision begins with the key point, but it’s often more effective to withhold significant information until the end of a sentence: â€Å"The more complex that adaptive systems are, the more quickly they can respond.† 2. â€Å"It’s the world of impressions, in which we can do no wrong, not the world of action.† In the previous example, the distinctions between the three variations of the statement were minimal, although the key point popped out more in the suggested revision. In this case, however, the statement begins vigorously, peaks in the middle, and limps to a close, and the key point is relegated to a parenthetical statement that separates two elements of a contrast. Reorganize the sentence by establishing the contrast and culminating with the conclusion: â€Å"It’s the world of impressions, not the world of action, in which we can do no wrong.† 3. â€Å"America should rely on the entrepreneurship and goodness of its citizens to be a great society, not on the well-intentioned but ineffective policies of government agencies.† Again, the heart of a sentence is buried in its middle, and the juxtaposition of the two approaches is interrupted. This sentence makes an assertion about how the United States can be a great society, and that final phrase should punctuate, not be buried in the midst of, the statement. Here are two variations that, as in the previous example, introduce a contrast and then ride the momentum of that tension to drive home the argument: â€Å"The United States should rely on the entrepreneurship and goodness of its citizens, not on the well-intentioned but ineffective policies of government agencies, to be a great society,† or â€Å"The United States should rely not on the well-intentioned but ineffective policies of government agencies but on the entrepreneurship and goodness of its citizens to be a great society.† Note how one variant is internally punctuated and the other isn’t. In the first alternative, the counterpoint, the phrase beginning with not, is parenthetical it could be omitted without affecting the coherence of the sentence. In the second revision, the two elements of the contrast are an integrated thought, and there is no parenthesis. If the second element of the contrast were omitted, the sentence would read, â€Å"The United States should rely not on the well-intentioned but ineffective policies of government agencies to be a great society† an incomplete statement. The pulmonary findings of subsequent radiograph in this patient, 3 days post-op resembles that of acute respiratory distress syndrome (increased interstitial pattern). No cardiac or pulmonary effusions were noted. This might have superimposed the findings of pneumonia and atelectasis secondary to mechanical ventilation. The third radiograph taken 4 days post operative shows decreased interstitial pattern suggestive of recovery. Post- surgery, the PO2 levels were very low, suggestive of hypoxemia. This can be expected in fat embolism (Weinhouse 2000). The PCO2 levels are also low suggestive of hyperventilation. The bicarbonate levels are slightly low, which is an attempt to compensate for the alkalosis. The overall picture of ABG is that of respiratory alkalosis. Following this, mechanical ventilation has improved the oxygenation which is evident in the subsequent ABGs. The pCO2 and bicarbonate levels have normalized and the pH is in the normal range. However the PaO2 levels have been fluctuating through out the first day post-op. The differential diagnosis mentioned is pulmonary thromboembolism, lipid embolus, myocardial infarction and pulmonary edema. The investigations reveal normal hemoglobin and platelet count, prolonged PT, normal LAP, normal urine analysis, normal liver function and renal function tests and normal cardiac profile enzymes. ABG is suggestive of severe hypoxemia and respiratory alkalosis, chest X-ray is suggestive of increased interstitial pattern and EKG showed no fresh ischemic changes. The EKG and normal cardiac enzymes rule out repeat myocardial infarction. The absence of Watermark’s sign in the chest X-ray rules out pulmonary embolism. However, the best method of ruling out this condition would be to do Doppler ECHO, rapid D- dimer tests and V/Q lung scan. PT/PTT would be normal in this condition. ABG would show fairly normal
